Our membership form and information about applying is available in a printable format(above). Please note that we do not currently accept online applications; you must print the application and mail it to the address indicated.

Once the application (plus appropriate fees and documentation) has been received, it will be forwarded to the Examiner.

The Examiner shall review the application and accompanying documents, after which the applicant will be notified that he/she must write an examination or that the documentation presented is sufficient for entrance to the requested level. A personal interview may also be required. In the event an applicant's application is not accepted, the applicant may appeal the decision to the CHA Executive within 30 calendar days. The decision of the Executive is final.

The Examiner may make recommendations to the level for which he/she thinks the applicant is qualified.
